In order to further provide patients and their family members with more convenient copy and mailing services of medical records, Zhongshan People's Hospital has simplified the application process and produce a special QR code for the copy and mailing services of medical records, which has been widely posted in wards, discharge offices and other places within the hospital.
From September 1, 2022, patients and their family members can choose the mailing service of medical records by scanning the QR code, filling in a form and submitting required materials instead of finishing these steps in person at the reception of the hospital's administrative service center.
ZSPH will send out the medical records 7 working days after the patients are discharged from hospital (excluding Saturdays, Sundays and holidays). Death records are not covered by the mailing service.
Patients can also call 0760-88611980 to cancel the mailing service at ant time.
